                    If you did not manage to obtain a copy of your medical and dental records prior to your departure, you
                    can request a duplicate copy.
 

                    Requests for medical records should be sent to:

                    Requests for dental records should be sent to:

                    Dental Directorate
                    NZDF
                    Private Bag
                    Wellington.

                    The following information will need to be provided with your request:

                                Full service description (include Maiden name if applicable)

                                Service Number
